How Regional Interoperability & Data Aggregation Create Unprecedented Care Outcomes

August 31, 2016 Suzanne Cogan

At a time when the survival of regional interoperability is in question, HealthInfoNet has found great success in implementing strategies across the state of Maine that have led to both sustainability and growth.

Watch this webinar to delve into how HealthInfoNet, a health information services organization founded in 2009 that operates Maine’s statewide HIE, has achieved growth and unprecedented outcomes for patients in a challenging environment.

This independent, nonprofit organization has leveraged technology strategies to engage with providers and patients and support advanced care models. HealthInfoNet leverages predictive analytics that assess patients’ socioeconomic status and how that impacts chronic illness risk.

The organization is believed to be the only health information services organization in the country doing this type of work, which will provide valuable information from a care management perspective and align with emerging provider requirements such as the Medicare Access and CHIP Reauthorization Act of 2015 (MACRA).

Presenters Shaun Alfreds and Suzanne Cogan detail HealthInfoNet’s data-driven approach to achieve remarkable outcomes in care coordination, transition and quality of care. They explain how their technology model and new type of analytics enhance clinical workflows and result in better patient outcomes.

Learning Objectives:

  • Describe how centralized aggregation, innovative analytics models and interoperability can improve care management
  • Summarize ways statewide interoperability is helping providers decrease costs by managing downside risk
  • Identify how analytics can leverage socioeconomic and behavioral health data with clinical outcomes to identify high-risk patients for preventive care and timely interventions
